The essentials

The four questions that come up

When should implants be changed?
For natural wear from ten years onwards, or to improve a result in terms of volume, shape or contours. A change that has occurred over time — capsular contracture, malposition, skin laxity — may also motivate a revision.
Can volume or shape be changed?
Yes. Round or anatomical implants can be replaced with the same or a different shape depending on the case. The volume can be increased by about 30% if you wish, or remain unchanged.
What if the implant has ruptured?
The replacement can be carried out in the same operation. If the rupture occurred before ten years, a claim under the manufacturer’s device vigilance scheme can be requested, examined case by case.
Can implants be removed without being replaced?
Yes: this is autologous conversion. Total conversion removes the implants and injects fat in their place. Partial conversion combines fat with a smaller implant.

When the question arises

Replacing breast implants is considered either for natural wear after ten years, or to improve or change a result in terms of volume, shape or contours.

The request may also stem from a change that has occurred over the years: typically a capsular contracture, an implant malposition, an asymmetry that has appeared, or skin laxity with sagging.

An assessment always precedes a revision

A revision is not a second placement: it is decided on the state of the pocket, that of the implants in place and the quality of the tissues. This assessment determines what is possible.

Replacing: shape, volume, pocket

Round or anatomical implants can be replaced with the same or a different shape depending on the case.

The volume can be increased by 30% if you wish, or remain unchanged. Sometimes a change of pocket may be necessary, or an adjustment of that pocket.

The original incision is generally reused wherever possible: this avoids adding a scar.

A revision is also an opportunity to review an old choice in the light of the implants available today — the ranges of profiles, gel cohesiveness and texturing have evolved.

A subject the surgeon teaches

Dr Azencot has spoken on breast implant replacement at the 35th SOFCEP congress (Versailles, 2023), on the treatment of the capsule according to implant texturing at the SOFCPRE round table “Breast oncology” (2022), and on European breast implant regulations at the ESAPS workshop (Brussels, 2025), of whose board and scientific committee he is a member.

Rupture and the manufacturer’s warranty

Replacement for implant rupture can be carried out in the same operation.

If the rupture occurred before ten years, a claim can be made to the manufacturer under its device vigilance scheme. It is examined case by case and covers the implants free of charge, as well as the clinic fees — according to the terms of the purchase contract for the implants with the manufacturer.

What should prompt a consultation

A late fluid collection around the implant, with swelling of the breast, should prompt a consultation. So should the appearance of an abnormal lymph node in the armpit. Annual follow-up, with ultrasound on request, remains the rule after ten years.

Converting to fat

Not every revision ends with a new implant.

Partial conversion consists of injecting fat and placing a smaller implant. Total autologous conversion consists of removing the implants and injecting fat in their place. Your surgeon will advise you.

This option appeals to patients who wish to lighten their breasts, to do without a foreign body, or whose tissues have changed to the point where an implant is no longer the most suitable choice.

Total autologous conversion is often possible in a single session, injecting in the region of 450 cc of fat per breast. A second session may be needed if the implant removed was large — beyond 350 cc — or if the reserves do not allow around 1 200 cc of fat to be harvested.

What can be combined

Sometimes the skin of the breast has loosened over time, and an associated mastopexy may be necessary.

Fat transfer can be used around the implants to enhance the cleavage or to camouflage the contours of the implant. It is the same procedure as in composite augmentation, applied here to breasts that have already been operated on.

Frequently asked questions

What patients ask

These answers reflect the content written and validated by the surgeon. Each one is written to stand alone, question by question.

Do implants have an expiry date?
It is not a matter of expiry but of wear: they last a minimum of ten years. Beyond that, annual ultrasound monitoring makes it possible to check their integrity.
Can I increase the volume on this occasion?
The volume can be increased by about 30% if you wish, or remain unchanged. The extent possible depends on the pocket and on the quality of the tissues.
Will there be a new scar?
The original incision is generally reused wherever possible, which avoids adding a scar. An associated mastopexy, if necessary, adds one.
Is a rupture covered?
If it occurs before ten years, a claim under the manufacturer’s device vigilance scheme can be requested. It is examined case by case and covers the implants and the clinic fees, according to the terms of the purchase contract.
Can I have my implants removed for good?
Yes: total autologous conversion removes the implants and injects fat in their place. Partial conversion combines fat with a smaller implant.
What is capsular contracture?
Capsular contracture is a hardening of the fibrous envelope that naturally forms around the implant. It is unpredictable and is one of the reasons for a revision.
Is an assessment needed before changing implants?
Yes. A revision is not a second placement: it is decided on the state of the pocket, of the implants in place and the quality of the tissues. This assessment determines what is possible.
Will the implant pocket need to be changed?
Sometimes: a change of pocket, or an adjustment of it, may be necessary. A revision is also an opportunity to review an earlier choice in the light of the implants available today.
Can loose skin be corrected at the same time?
Yes. When the skin of the breast has loosened over time, a breast lift can be combined with the replacement.
Can fat improve the result of an implant change?
Yes: fat transfers around the implants make it possible to work on the cleavage or conceal the edges of the implant. It is the technique of composite augmentation, applied to breasts that have already been operated on.
How many sessions for a total autologous conversion?
It is often possible in a single session, injecting around 450 cc of fat per breast. A second session may be needed if the implant removed was over 350 cc, or if the reserves do not allow around 1 200 cc of fat to be harvested.
What signs should prompt a consultation?
Swelling of the breast due to a late fluid collection around the implant, or the appearance of an abnormal lymph node in the armpit. After ten years, annual follow-up with an ultrasound check on request remains the rule.
What anaesthesia and what hospital stay?
The operation is performed under general anaesthesia and lasts 1 h to 1 h 30 min, as day surgery or with one night in hospital. Activities are resumed after about 5 days.
